Moving Towards Wellness – Chronic Disease Self Management is a self-management program for people living with chronic conditions and is offered by COTA Seniors Voice with funding from the Department of Health.
The program aims to enable people with chronic conditions to manage them by providing information and teaching techniques to deal with the issues associated with these conditions.
COTA Seniors Voice has a number of volunteer peer leaders who are themselves living with a chronic condition and have been trained by Master Trainers from the Spencer Gulf Rural Health School. These peer leaders are available to work with organisations who are licensed to conduct the Stanford model of chronic condition self management.

Your time commitment is about two and a half hours per week.
People who commit to the program will receive a free copy of the book Living a Healthy Life with Chronic Conditions edition 3
